Liability waiver

BAP members will receive the liability waiver electronically before attending community service events.

If you are not a BAP member or if you have complications filling out the electronic form, complete this PDF and send the it to Communityservice@unlvbap.org

Failure to submit the waiver will preclude you from attending the event and earning hours.
